Personal injury collision #8/2016, Fail-to-remain, 48 Milldock Drive

On Friday, January 28, 2016, at 9:30 p.m., police responded to a call for a man found on the road at 48 Milldock Drive .

It is reported that:

- a 59-year-old man was found lying on the road at the foot of the driveway of 48 Milldock Drive

- the man was taken to hospital with life-threatening injuries

- medical investigation has determined that the man was struck by a vehicle

Police are seeking the public's assistance, and are asking for any video recordings of the area on January 28, 2016, between 8 p.m. and 9:30 p.m.

"Abuse it and lose it," Accessible Parking Permit enforcement campaign, Monday, February 8, 2016 to Sunday, February 21, 2016

The Toronto Police Service is committed to assisting people with disabilities in the community.

On Monday, February 8, 2016, TPS Parking Enforcement will launch a heightened enforcement campaign entitled, "Abuse it and lose it." The campaign will focus on the enforcement of accessible parking spaces, the misuse of Accessible Parking Permits, and the detection and retention of counterfeit Accessible Parking Permits.

The campaign will run until Sunday February 21, 2016. Parking Enforcement Officers, members from Traffic Services and Divisional units will be assisting in this campaign. Areas of the city where significant problems have been identified for the misuse of accessible permits will be included in the directed patrols.

The City of Toronto grants many parking bylaw exemptions to holders of Accessible Parking Permits, in addition to providing designated on-street parking spaces for people with disabilities. The abuse of these exemptions deprives legitimate accessible permit holders from acquiring parking spaces that are safe and accommodating for their needs.

In 2015, TPS Parking Enforcement seized over 1,000 Accessible Parking Permits and charged over 900 people under the Highway Traffic Act for misuse.

Members of the media are welcome to participate in ride-alongs with members from Parking Enforcement engaged in this campaign. To arrange a ride-along, please call Brian Moniz at 416-808-1641.

1. two counts of Attempt Break-and-Enter

He appeared in court at Old City Hall on Thursday, January 21, 2016, room 101.

Kathleen Adams, 91, was last seen on Friday, February 5, 2016, at approximately 8:20 a.m., in the Kingston Road and Bellamy Road area.

She is described as 5'4", slim build, white hair, blue eyes and uses a black walker. She was last seen wearing black pants, a white sweater, and white sandals.

She was also seen entering the passenger seat of a red, Toyota Rav 4, driven by an unknown woman.

Octavia Kelsie, 12, was last seen on Saturday, February 6, 2016 , at 2 p.m., in the O’Connor Drive and St. Clair Avenue East area.

She is described as 5'5", with a skinny build, and long black hair in cornrows. She was last seen wearing a waist-length winter coat with fur on the hood, purple low-cut Converse shoes, a knitted hat with brown and burgundy stripes. She's may be wearing snowman earrings.

She may be with her small dog.

Three arrested in automobile insurance fraud investigation

The Toronto Police Service would like to make the public aware of an investigation involving alleged fraudulent automobile insurance claims.

It is alleged that:

- people who were involved in motor vehicle collisions attended for treatment at the Wellness Centres of Ontario, at 3500 Dufferin Street

- the centre then invoiced an insurance company for treatments that were never rendered

It is further alleged that:

- a paralegal from Kovtman Law, at 3500 Dufferin Street, assisted clients in filing fraudulent accident benefit claims against the insurance company

On Friday, February 5, 2016, police arrested three people.

Dr. Edward Hayes, 59, of Vaughan, was charged with:

1) three counts of Fraud Under

Doina Osacenco, 57, of Toronto, was charged with:

1) three counts of Fraud Under

Paralegal Anna Kovtanuka, 31, of Thornhill, was charged with:

1) two counts of Fraud Under
2) Possession of Property Obtained by Crime Under

All three are scheduled to appear in court at 1000 Finch Avenue West on Thursday, March 17, 2016, 1 p.m.

Police warn public about rise in identity thefts after fraud lab discovered, Protect your mail, shred your documents

The Toronto Police Service would like to inform the community about a fraud lab discovered as a result of a lengthy investigation which included mail theft and countless identity thefts at local Money Marts in Toronto.

A number of items were discovered among the technology and computers in the Bloor Street East and Parliament Street area.

Police would like to warn the public about growing concerns, due to mail theft, that have resulted in these types of identity theft investigations.

Police want to stress that theft of mail has become an issue within the Greater Toronto Area. The Beach area of the city is one of the targets of this type of fraud and it is imperative that the community is warned about these predators.

The items discovered in these types of labs can include the following:

- financial statements, bank documents and cheques stolen from the mail
- fake driver's licences generated and created in the victims' names
- fake bank statements generated
- fake SIN cards
- fake government identification
- bills and tenancy documentation stolen from the mail
- pay cheques, blank cheques and direct deposit statements
- student loan documents and bank loan documents
- T4 slips, tax returns and medical documents

Police want the public to be vigilant with their mail and to shred everything prior to disposal.

Protect yourself accordingly. We put blinds on our windows, locks on our doors but, most importantly, we need to protect our identification. Put a lock on your mailbox and shred the items that expose your name and identifiers.

Identity theft has become the crime of today.
