On Tuesday, August 13, 2013, at approximately 11:30 a.m., police responded to a personal injury collision at Steeles Avenue East/Middlefield Road.
It is reported that:
- a 40-year-old man was driving a 2010 Hino truck eastbound on Steeles Avenue East, approaching Middlefield Road

On Friday, August 16, 2013, Vincente Bacobe Arbis, 40, of Toronto, was arrested.
He is charged with:
1) Criminal negligence causing death
2) two counts of Criminal negligence cause bodily harm
3) Dangerous operation causing death
4) two counts of Dangerous operation causing bodily harm
5) Dangerous operation
He is scheduled to appear in court on Friday, August 16, 2013, at 1911 Eglinton Avenue East, in courtroom 412.
